Consent to the processing of personal data for the purpose of direct marketing

WE CARE ABOUT YOUR DATA

By ticking the “checkbox” or by filling out the “application form”, I confirm that I have familiarized myself with how Traxlo, UAB, legal entity code 305546038, registered address Saulėtekio av. 15, LT-10224 (hereinafter – the Company), as a data controller, will process my personal data under the conditions specified below, and I freely consent to the processing of my personal data as follows:

Sending of direct marketing messages (newsletters, commercial offers).

PROCESSED PERSONAL DATA

Identification data: name of the person.

Contact data: e-mail, telephone number.

 

I HAVE BEEN INFORMED ON THE FOLLOWING:

  1. the Company shall process my above personal data based on the legal basis of my freely expressed consent and I have the right to withdraw it at any time;

  2. when processing my personal data for above-mentioned purpose, the Company shall not use automated decision-making processes, including profiling, of such nature which could have legal consequences for me or have similar significant effects;

  3. the Company is entitled to obtain my above personal data only from me;

  4. the Company is entitled to transfer my above personal data to the data processors, which provide services of direct marketing message sending, information technology, and data storage;

  5. the Company shall process my personal data for as long as my consent is valid, but no longer than 2 (two) years. If I withdraw my consent for the processing of personal data for the purpose of direct marketing, the personal data shall no longer be processed for that purpose and will be deleted.

I have been informed and understand that I have the following rights: to familiarize myself with my personal data being processed; to demand correction of my incorrect, incomplete, or inaccurate personal data; to demand the deletion of my personal data or the termination of my personal data processing actions, with an exception of storage, if such actions are carried out in violation of the law; to obtain my personal data in a structured, commonly used and (or) computer-readable format; to object to the processing of my personal data; to withdraw this consent for the processing of my personal data, without affecting the legality of the data processing based on consent prior to its withdrawal; to limit the processing of my personal data according to applicable laws; to file a complaint with the State Data Protection Inspectorate.

I have been informed that withdrawal of consent shall not affect the legality of consent-based data processing prior to the withdrawal of consent.
